Making Your First Visit a Success
Welcome to Palm Leaf Dental! Our team is ready to help you enjoy life with a healthier, more radiant smile, but before we can start, we would like you to review the following information. These details will ensure that your first visit to our Nocatee dental office is a success. You’ll learn what to expect when arriving as well as the different payment options we readily accept. Please feel free to call us if you have any questions.
Your First Visit
Your first visit will begin with a warm welcome from one of our team members before being escorted to a treatment room. There, we will take full X-rays so that we can get a complete view of your oral anatomy before one of our hygienists begins the periodontal charting process. This measures your gum tissue depth to determine if gum disease exists. During your appointment, we’ll review your X-rays and discuss our findings before performing a thorough cleaning of your teeth. We’ll then take intraoral photos of the inside of your mouth, spotlighting any teeth that may need additional care. Dr. Veazey-Georges will then come in to perform a complete examination and discuss a proposed treatment plan as well as additional appointments and fees.
Dental Patient Forms
Your time in our office shouldn’t be spent filling out new patient paperwork. This is why we make it easy to click on the link below and complete the information from the comfort of your home. Just be sure to bring the finished forms with you to your appointment for a more seamless check-in.

Dental Insurance
Our team is pleased to welcome your dental insurance and is in-network with MetLife, Guardian, Ameritas, and Principal. Not only will we help to maximize your available benefits, but we’ll also file any claims on your behalf as a courtesy. We believe in creating a stress-free experience for our patients, and this is one way we can ensure your focus remains on your oral health and not only the financial obligations that are required.
CareCredit Financing
If you are recommended for a treatment that is not covered by your dental insurance, consider applying for CareCredit. This third-party financier makes it easy to pay off your total bill over time through manageable monthly installments. With low- and no-interest plans available, you can pick one that is right for you and your budget.
Cancellation Policy
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24-hour notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
